Patient Forms

The following information and items will allow Dr. Solman to treat your condition more efficiently. He appreciates your cooperation during this very important process. 

To save you some time on your initial visit we have provided patient registration forms to download and complete. You will also need your driver’s license and insurance cards. X-rays will be taken if you do not bring any with you as Dr. Solman will want to review them before discussing your condition. Any other diagnostic studies such as MRI’s or X-rays taken by previous physicians can also be helpful with diagnoses and signs of progression of osteoarthritis conditions. Please bring any previous studies with you to the visit. Bring a list of medications that you are currently taking, along with all dosages. We welcome any spouses and other family members who wish to participate in the discussion of your treatment to also attend the examination with you.
